Community Day Nursery offers healthy meals to all enrolled children as part of our participation in the U.S. Department of Agriculture’s (USDA) Child and Adult Care Food Program (CACFP). CACFP is a federally funded program, which provides reimbursement to Registered Child Care providers for the healthy meals that they serve to the children in their care.  The CACFP aims to assist childcare providers to serve nutritious meals in a setting, which promotes and encourages positive eating habits. Please help us comply with the requirements of the CACFP by completing the attached Household Income Eligibility Statement (HIES) form and return to our office. This information will be kept strictly confidential in our files.  Only one completed HIES is required for all children in your household. Once we have properly approved your HIES as eligible, our agency will receive the higher (“Free” or “Reduced-price”) meal reimbursement rates for your enrolled children, for 12 months from the Effective Month of Determination regardless of any change in your household size and/or income or termination from Benefits Programs.
